Pediatric UTI

Overview

Urinary tract infections in children can present differently than in adults, particularly in infants, and always warrant evaluation for an underlying cause, especially after a first infection.

Symptoms

Fever without an obvious source, especially in infants
Pain or burning during urination
Frequent urination or new-onset bedwetting
Foul-smelling urine
Poor feeding or irritability in infants

Causes

Bacterial infection is the direct cause, often linked to underlying anatomical factors such as vesicoureteral reflux in children with recurrent infections.

Risk Factors

Female gender (after infancy)Girls are more prone to UTIs after early infancy.
Uncircumcised male infantsCarry a somewhat higher UTI risk in the first year.
Vesicoureteral refluxA common underlying factor in recurrent childhood UTIs.
ConstipationCan contribute to incomplete bladder emptying and infection risk.

Diagnosis

A urine test and culture confirm the infection; imaging such as ultrasound or a VCUG is considered after a first UTI in young children or with recurrent infections.

Complications If Untreated

Recurrent or delayed-treatment infections raise the risk of kidney scarring.

Prevention Tips

Good hygiene, treating constipation, and prompt treatment of infections all help prevent recurrence.
